Abstract: | Based on data from the World Health Organization (WHO) in 2015 the Maternal Mortality Rate (MMR) around the world is estimated at 216 / 100,000 live births and the Neonatal Mortality Rate dropped 47% to 19/1000 live births. Efforts to accelerate the reduction of MMR are carried out by ensuring that each mother is able to access quality maternal health services, such as maternal health services, delivery assistance by health personnel, postpartum maternal and infant care, and family planning services. Therefore, Continuity of Care is carried out. After doing midwifery care to Mrs. S there is no problem during the pregnancy. The birth process of babies born spontaneously with Birth Weight 2900 grams, Birth Height 50 cm, apgar score 9/10, with male sex, found second degree perineal rupture problem during the labor. Care for newborn has been given as needed and there are no distress signs or complications in infants. During the last visit of the postpartum period, information about Family Planning use was informed, the mother decided to become a 3-month contraceptive injection acceptor. |
