Abstract: Background: High maternal mortality rate in indonesia is caused by several faktors, one of which is the third trimester prignancy discomfort. If not given proper midwifery care,it can cause pregnancy complication. Objective: Improve the knowledge and ability to equip ourselves with science and skills to give The midwifery Continuity of care in pregnant, labor, puerperium, neonatal and family planning use the midwifery management. Method: Continuity of midwifery care management with Subjeck Objeck Assagment Planning management. Results: After midwifery care is done to Mrs.R there is a problem of discomfort that mother feels and discomfort mothers doesnot in danger the mother and foetus. The maternity is held during the 40th weeks. The baby borns normally labor presipitatus with 3.500 gr of weight, 50 cm of height, 8/10 of apgar score, and sex of female. There is a second degree perineal rupture happened during the process. Four stitches are applied on the rupture using catgut. In childbirth period, there is not any danger found that the process of lactation and involution runs well. In new born baby period, there is no complication found, and after doing a consoling, Mrs.N chooses to be a family planning acceptor for 3 months. Conclusion: Every cares that have been given runs normally. Hance, the attempt to raise the quality of expectant health is by doing a comprehensive and continuity care in order to lower down the number of Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR).
