ASUHAN KEBIDANAN PADA NY. R MASA HAMIL, BERSALIN, NIFAS, NEONATUS DAN MENJADI AKSEPTOR KB DI KLINIK BIDAN E.S KOTA PEMATANGSIANTAR

Abstract

Background : Death rate of mother and baby is barometer to evaluate the health degree of Nation. Mother health must be started when a mother prepares her pregnancy, as far as pregnant time, giving birth, in puerperium and gestational time, as far as using the family planning acceptor until having productive/ old age. Purpose : The purpose is to increase the health mother and baby by applying the continuity of care, starting from pregnancy time to becoming an acceptor KB program. Method : The method used is continuity midwifery care and documenting by using Subjective Objective Assessment Plan management. The results : Mrs R, is 30 years old, Multigravida woman (Gravida II), Giving birth 1, Abortion 0. The last menstrual period was in July, 20th 2017. The estimated date of birth is in February, 20th 2018. Mrs R pregnancy examination was performed 3 times, HB 9,8 gr%, consuming Fe tablet one time in a day. Maternity for Mrs R happens in 40 week pregnant age, Efficiency of Breast Crawl was succed and it has no problem. The baby born normally with body weight 3300 gr, body length 50, female gender and Appearance Pulse Grimace Activity Respiration 8/10. The baby got colostrum. Umbilical cord coils is loosed in 7 days. In puerperium it is engorgement. After getting breast care, the baby want to get breast milk. And Mrs R became is a three-month injection family planning acceptor. Conclusion : Implementing concept of midwifery care is conducted to Mrs R with continuity of care, it is hoped to be barometer and basic direction in performing midwifery service.
