Abstract

Back ground of problem : The success of maternal and child healt efforts can be seen from maternal and infant mortality rate. Based on report from profiles of the district/city Maternal Mortality Rate reported in Sumatera Utara year 2014 is 75/100.0000 live births. And based on population census in 2014, infant mortality rate in Sumatera Utara is 20,22/1.000 live births. Purpose : To improve maternal and child health to give services sustainably and continuity of care to Mrs. E period pregnancy to become a family planning acceptor in accordance with the standards and authority of the midwife. Method : Continuity midwifery care and documentation with SOAP(Subjektif, Objektif, Asessment, Planning) management. Result : Mrs. E was 37 years old, secundigravida, Gravidarium III, Giving Birth II, Abortion 0, gestational age is 12 weeks. The last menstrual period was in June,16th 2017,and the estimated date of birth is in March, 23th 2018. This can be overcome with a total bedreas. The process of labor is normally with, the weight 3400 gram, the body length 49 cm, the head circumference 34 cm, the chest size 33 cm, the arm circumference 11 cm, Apgar score 9/10, women, there is no congenital anormalies. During childbirth, she had rupture of the perineum with grade I. At the time of post natal care there is no problem found. The neonatal care is given according to the benefit and found no sign at danger or complication that occur in neonatal. On the last visit during delivery has been informed about the use of contraception, and the mother decided to become family planning acceptor Implant and mother to decide the use of contraception Implant. Conclution : In the case of Mrs. E, 37 years old with rupture perineum grade I. There is no gap between theory and practice. Services and care provided in accordance with standard of care and authory of the midwife.
