Abstract: AKI of North Sumatra amounted to 328 / 100,000 KH, but still quite high when compared to the national figure of the 2010 SP results which amounted to 259 / 100,000 KH. While based on the results of the AKI and AKB Survey conducted by the North Sumatra Provincial Health Office, it was 268 per 100,000 live births. This report aims to enable the authors to implement sustainable midwifery care for mothers from the period of pregnancy to family planning according to the standards of care and approach to midwifery management. The method used is case studies and documentation with SOAP management (Subjective, Objective, Assessment, Planning). Midwifery care Ny. F 28 years old, GII PI A0 has problems with anemia and perennial rupture. In the theory of anemia in perenium rupture pregnancy can lead to bleeding and death when in postpartum and during childbirth but in practice when postpartum and postpartum period the mother has normal bleeding. In the case of Mrs. F with anemia and uterine rupture there is no gap between practice and theory. Mothers are encouraged to consume nutritious foods, Fe tablets and become family planning acceptors
