Abstract

Background: Health care can improve the health of the people. One of them is seen as having an important role is health care. The ability of health care of a nation is measured by determining the level maternal and perinatal mortality in 100.000 live births, Maternal Mortality Rate (MMR) and infant Mortality Rate (IMR) is high. This report aims to allow the authors to apply continuity of care to mothers from the time of pregnancy to becoming a family planning acceptor in accordance with the standard of care. Method: This report was prepared by using midwifery management with SOAP method. Result: Mrs. D is 30 years old, 28 weeks gestation age on antenatal care and Hb maternal examination 10,5 gr% and antenatal care examination 8 times. Nipple examination 4 times, infant examination after birth 3 times and given family planning service. Application of antenatal care has been found to be anemia problems in pregnancy and has been given Fe tablets and nutritous food containing iron. The delivery went well and the mother had 2nd degree perineal rupture, done effeciently of Breast Crawl for 60 minutes, newborn baby received normal newborn care, and baby with spit. The postpartum period is going well without any problems. The short-term family planning method is 3 months injections with Depoprovera. Conclusion: Applying of midwifery care pattern is to improve health quality for pregnant woman with anemia has been overcome well, labor with rupture perineum of degree 2 has been overcome well, period of puerpery did not have problem, baby with spit overcome well and mother has used contraception tool
