Abstract: Backgrounds : Maternal and child health are things that needs to get priority in the implementation of health efforts, mother and child are vurnerable groups to the family and surrounding condition in general, so an assessment of health status and performance of maternal and child health efforts is important to do. Purpose : To improve the maternal and child health status by applying continuity of care on pregnancy, maternal, postnatal, newborn and family planning. Method : Midwifery continuity of care and SOAP management Result : Mrs. Y (30 y.o), G11P1A0, HPHT 10-06-2017, 3 visits, experienced physiological anemia at 28-30 weeks, can be solved by taking Fe once a day. In laborn Mrs. Y are experienced rupture perineum grade I, and no problems found. The baby was born spontaneously weight 3200 gr, height 48 cm, female sex, apgar score 9/10. The umbilical cord is loose at day-6 and gets the breast milk. In the postnatal period, Mrs. Y has no any probem, breasfeeding is smoothly happen. In the Newborn baby care there is no complication, weight 3200 gr, height 48 cm, head circumference 34 cm, chest size 35cm, apgar score 9/10, female sex and Mrs. Y becomes Family planning acceptor of 3 month injection type. Conclusion : The application of mindset to continuity of care taken on Mrs. Y are expected to be a benchmark or guidelines to give midwifery care
