dc.description.abstractObjective: Provide Continuity of Care midwifery care for pregnant, maternity, postpartum, and family planning mothers with care standards and use SOAP documentation with a midwifery management approach. Method : Case report Results: Based on the health profile of the district / city, maternal mortality in North Sumatra in 2016 was 85 / 100,000 live births. Where as the baby’s death rate is 4/1000 live births. Mrs. F is 30 years old, GII P1 A0 with 28 weeks' gestation. The first day menstruation last (HPHT) : 02-04-2017, and the calculations date of labor (TTP) : 09-01-2018. On Ante Nata Care implementation in Mrs. F only found physiological problems during pregnancy and all can be resolved. In the pregnancy of Mrs. F experienced anemia, but this can be overcome by giving Fe tablets so that the labor process runs smoothly. At the age of 40 weeks, 14.50 pm, the woman came to the midwife's clinic with complaints of mucus mixed with blood, and the examination was done in fact when I was in the active phase with an opening of 5 cm. At 18:00 pm, the opening day was 10 cm complete and there were signs and symptoms at the second stage, and the membranes were still intact and supported by maternal support. Mother is accepting well. At 19:00 pm., the baby was born normal with male gender, weight 4000 grams, Long body (PB) 50cm, The size of head (LD) 33cm, The size of the chest (LD) 36cm, Apgar Score 9/10 babies immediately cried. At the time of Kala II delivery there were no problems. In the implementation of care during III Ny. F has a degree I rupture with a knot technique. At Newborns care for Mrs. F not found complications and Mrs. F becomes a 3 month injection family planning acceptor. The Conclusion: The application of midwifery care that was done to Mrs. F with Continuity of Care is expected to be a benchmark and guide in providing midwifery services.en_US
