ASUHAN KEBIDANAN PADA Ny.N MASA HAMIL, BERSALIN, NIFAS, BAYI BARU LAHIR, KELUARGA BERENCANA DI BPM D KOTA PEMATANGSIANTAR

Abstract

Caused of increase mother mortality are pregnancy and maternity coplication, so we must early to know the pregnancy and maternity complication, and the time post natal have to given mother mortality rate because the time post natal mother easily come into contact infektion. Report aims to improve maternal and child health to give services sustainably and continuity of care to pregnant mother until it become a family planning acceptors and it must be corresponding with standard and authority midwife. The method used is continuous midwifery care and authority midwife by using some documenting case report with SOAP method. Mrs. N was 33 years old, GII PI A0 gestational age is 12 weeks, HPHT April,15th 2017, Estimated delivery data TTP January, 22th 2018. She had checked her pregnancy about one time and feel her fetal movement at 16 weeks. During pregnancy, from the antenatal care found mother with mild anemia, but in the end of the pregnancy the mild anemia of mother can be handled properly. then the baby was born spontaneously with male sex, the weight 3500 grams, the body length 53 cms, Apgar score 10/10, there is no congenital anormalies. During childbirth, she had rupture of the perineum with the one-degree. At the time of post natal care there is no problem found. The post natal of Mrs. N is running normally without complications. The last visiting given counseling to Mrs. N about family planning and Mrs. N choosed to be an acceptor injection 3 months of family planning. Midwifery care gave from gestation become to an acceptor of family planning according to the standard of midwifery care and authority of midwife. If the patient has a complaint, immediately come to the nearest health worker
