Abstract

Background of study Maternal mortalitas and morbility still fregnently become health Problems in developing Countries.According to World Health Organization (WHO) report in 2014.Maternal Mortality Rate (AKI) in the world were 289,000 lives.Some Countries had high enough AKI Such as Sub –Saharan Africa 179.000 South Asia Were 69.000 live,and southeast Asia 16.000 lives.Maternal mortality rate in southeast Asian Countries like Indonesia were 190 per 100.000 Live births,Vietnam are 49 Per 100.000 Live births,Thailand were 26 Per 100.000 Live births,Brunei were 27 Per 100.000 live births,and Malasyesia were 29 Per 100.000 live births (WHO,2014). Maternal dcath was the death of a woman which occurred during Pregnancy,Childbirth,Or 42 after aclivery with a direct or indirect cause of Childbirth. Objektive : Applying midwifery nurture continuity of care to mothers,pregnant Period recome a KB acceptor according to mid wifery care standard. Method : Mrs.E 35 years old <Gravida 4,Parity 3,Aborttion 0 Pregnancy age in the second trimester.The first contact of gestational age enters 14 weeks.The result of Hb Check Up Was 9,8% and mother had a mild anemia.Evalution at the visitation of the 23 weeks of Pregnancy was found that the mother was still anemia with Hb was 9,4 g%.The Suggestions Was often having dihoner in small portions and avoded foods that in small portions and avoided foods that invited a nausea and consumes nutritions foods which containes carbohydrates, Vitamin,iron and folic acid.on Mrs .E is 35 years old.Gravida 4,Partus 3,Abortuse 0 with Pregnancy age 38 At the periode of pregnancy there was not a complication to the baby howener the mother had a ruptured perineum degree 1 and she was immediately compressed with anti septic fluid.Monitiring was done during the Puerperineum to assess the normality of the process was undergone to become a family planning acceptor.Family Planing method which was choson by hormonal mother was injecting 3 months.The infant eondition from birth til the end from neonatal period was monitored to see the Possibility of problem which happened. Conclusion : The implementation of midwifery nurture conducted to Mrs.E with a continuity of care could improve maternal and child nealth
