Abstract: Maternal and infant mortality rate is a benchmark in assessing the health status of a nation, therefore the government is very stressed to reduce maternal and infant mortality through health programs. To improve the degree of maternal neonatal health is applied continuity of care midwifery care to pregnant women, childbirth, childbirth, newborn and family planning in accordance with standards of care and using the SOAP method. Midwifery care standards are a reference to the decision-making process and actions undertaken by the midwife in accordance with their authority and scope of practice based on midwifery science and tips, the formulation of diagnoses and / or obstetric problems, planning, implementation, evaluation and recording of midwifery care. The method used is continuous midwifery care and documentation with SOAP (Subjective, Objective, Assesment, Planing) management. Mrs.M 31 years old, GIII PIII Ab0 HPHT: 20-04-2017, TTP 27-01-2018. In the implementation of the ANC only found a physiological problem and all can be solved. Labor on Mrs.M takes place at 38 weeks' gestation. In nymat childbirth care no problem was found, so the lactation and involution process went smoothly. On BBL Mrs.M treatment, no complications were found and Mrs.M became an injection acceptor. Of all the care given from the pregnancy until the family planning acceptors in accordance with the standards of care and authority of midwives. So it can reduce Maternal Mortality Rate and Infant Mortality Rate
