Abstract: Background : Maternal mortality rate and baby mortality rate in north sumatera province still relatively high when compared to other provinces. North sumatera province become the 6 th province with highest mother’s rate mortality in Indonesian. Based on reports from district and city profiles maternal mortality rate on North Sumatera on year 2012 more than 106 compared to 100.000 live births. Object : Given service midwifery continuity of care starts from beginning of pregnancy until becomes as an acceptor family planning. Method : Study case method and documentation with SOAP management. Result : Mrs. E aged 33 years old is a secondgravida woman (gravida II), giving birth I, abortion 0, The last menstruation period was June, 02 nd 2017 and the estimated day of birth is March, 09nd 2017. Antenatal care from Mrs. E is found some inconvenience problem on pregnant mother, there are anemia, back pain on trimester II and frequent urination on trimester III. The process of labor is spontaneously, babies born healthy with weight 51 cm, female gender. The baby has received normal newborn care, no danger sign or complication. During the postpartum period is physiological and the care provided is good without no danger sign. Mother decided to become family planning acceptor injection for three months. Conclusion : The care provided from pregnancy to becoming a family planning acceptor is in accordance with the standard of care and authority of the midwife
