Abstract

Maternal and infant mortality rate is benchmark in assessing the health status of a nation. Therefore ,the government is very stressed to reduce maternal and infant mortality through health programs .To improve the maternal degree of neonatal applied midwifery care continuity of care in pregnant women, born childbirth newborn and KB in accordance with the standard of care and using SOAP method. Midwifery care standards are a reference to decision making processes and action undertaken by midwives in accordance with their authority and scope of practice based on midwifery science and tips, the formulation of diagnoses and midwifery issues, planning, implementation, evaluation and recording of midwifery care. Method that using is midwifery care sustainable and documenting with SOAP management. Ny. I 35 years old, GII PI Ab0 HPHT: 25-05-2017, TTP 03. On implementation ANC only found a physiological problem and all can be solved. Childbirth Ny. I. take place at 38 weeks gestation. In childbirth found rips level I but rips no need to stitched. On care during Ny ’s childbirth no problems found, so lactation process and involution smoothly. At 19.50 wib baby born spontaneously with male gender, BB 3200 gr, PB 49 cm, LK 34 cm, LD 33 cm, Lila 11 cm, Apgar score 8/10, there is no congenital aberration. On care BBL Ny. I not found complication and after give counseling Ny. I choose became acceptor KB 3 month. Over all care that give from pregnancy period until acceptor KB suitable with standard care and authority of midwife. So that it can reduce maternal mortality and infant mortality
