Abstract: | Maternal and child health is one part of efforts to improve health degree in Sustainable Development Goals (SDGs) Indonesia 2030. Indicator of the target achievement is if Maternal Mortality Rate (MMR), Neonatal Mortality Rate (AKN) and Infant Mortality Rate (IMR) decreased As well as Family Planning acceptors (KB) increased. The physiological mothers' condition during pregnancy is expected to continue until the postpartum period ends. This report aims to apply continuity midwifery care to mothers from pregnancy to family planning according to the standard of care and approach to midwifery management. The method used is study case and documentation with SOAP management. Mrs D is 23 years old, secundgravida, giving birth I, abortion 0. Mothers with moderate anemia in the third trimester pregnancies were treated with administration of Fe tablets and returned to normal at 39-40 weeks' gestation. At the time of delivery there is perineal rupture of garade II and sutures with a knot technique using cut-cut yarn. The postpartum period is normal. The new born baby with weight: 3300 gr, the body lenght: 48 cm, apgar score 8/10, gender: male. Mrs D becames acceptor of 3 month injection of family planning. The implementation of midwifery care mindset that is done to Mrs D with continuity of care can improve maternal and child health |
