ASUHAN KEBIDANAN PADA NY. S MASA HAMIL, BERSALIN, NIFAS, BAYI BARU LAHIR, SAMPAI DENGAN KELUARGA BERENCANA DI KLINIK BIDAN RM KOTA PEMATANGSIANTAR

Abstract

Backgrounds: Based on the result of the population cencus in 2015, AKI in Indonesia are 359/100.000 KH. The report profiles in 2014, 1.236 babies died before turning 1. This figure can be taken into account, AKB in North Sumatera are 10/10.000 KH on 2014. Purpose: To provide care in obstetrics on Mrs. S ( 23 years old) with continuity of care started from pregnancy, maternity, newborn and family planning acceptor in accordance with the standards care of midwifery and midwifery management. Method: Midwifery continuity of care and documentation by SOAP management. Result: After midwifery care to Mrs. S was found Mild Anemia, but it was handled with improvement of the pregnancy nutrients. In the theory of pregnancy with anemia can lead to bleeding and deaths during maternity and postnatal. The midwifery of care on Mrs. S pregnancy with mild anemia does not lead to complications in pregnancy and maternity. The baby was born spontaneusly with the weight 3300 gr, length 49 cm, apgar score 8/10 with female gender. In the implementation care of kala II, Mrs. S experienced rupture but the problem can be resolved by doing hecting with baste technique. The care on a newborn was given according to the needs of baby’s. In the recent postnatal visit, has been informed about to use a contraception. The patient decided to becomes family planning acceptor of injection one time in three months. Conclusion: In the case on Mrs. S (23 years old) with mild anemia, and rupture perineum, it is found no gap betweeen theory and practice. The patient is recommended to consume nutrious foods, Fe tablet, and be a family planning acceptor
