Abstract: Based on the result of the population cencus in 2010, AKI in North Sumatera are 328/100.000 KH. The results of population surveys among the 2015 census shows an IMR of 22,23/1.000 KH. AKB in Pematangsiantar city in 2016 increased to 6/1.000 KH. To provide care in obstetrics at Mrs. N age 39 years old with continuity of care started from pregnant women, maternity, childbirth, and newborn and acceptor After midwifery after care to Mrs. N, there is a problem anemia. In theory of pregnancy with anemia can lead to bleeding and deaths during childbirth and puerperium. Care in midwifery in a sustainable manner of Ms. N pregnancy with mild anemia does not lead to complications in pregnancy and chilbirth. The baby was born naturaly with the weight of 3900 gr, the length of 50 cm, apgar score 8/10 with female gender. In the implementation of the care kala II, Mrs. N rupture but the problems can be resolved by doing hecting with baste technique. Care on a newborn baby given according to the neeeds of baby’s. In a recent visit of the childbirth, has been informed about the use of contraception. The patient decided to become acceptor family planning injection one time in three months. In the case of Mrs. N 39 years old with emesis gravidarum, mild anemia, and rupture perineum, it is found no gap betweeen theory and practice. The patient is recommended to consume nutrious foods, Fe tablet, and be acceptor family planning.
