In this case the patient had mild anemia with HB 10,7 gr%. Suggestion often eat in small porsions and avoid foods that invite nausea and consume nutrision foods that contain carbohydrates, vitamins, iron and folic acid. with a fast and pracise treatment is expected pregnant women are not experiencing the potential of moderete or severe anemia. The purpose of this peper is for the outhors to gain real experience and provide midwifery care to Mrs. R GII PII A0 continuity of care starting from pregnancy, childbirth, newborn up to familly planing acceptor. in this case, the outhor has given care to Mrs. R Starting from september 10 august 2017 until 10 april 2018 with the method of documenting SOAP (Subject, Object, Assement, Planning) in accordance with the standart of Midwifery care. The methods used in data collection are interviews, physical examination and observation. After the Midwifery care doing to Mrs. R 32 years old, Gravida 2, Delivery 1, Abortion 0 with 38 weeks of gestation. When the deliveri’s time, the is complication with Msr.R the complication is Ruptur of the perenium level 2 and there iis no complication wih baby and normal baby care is done. Monitoring is done during the puerperium to assess the normality of the process experienced to become a family planing acceptor. Family planing method choose Mrs.R of short method is injection and done by injection KB 3 month. The state of the beby from birth to complication from neonatal period is monitored to see the possibility of problems that occur
