Abstract

Back ground of problem : The success of maternal and child healt efforts can be seen from maternal and infant mortality rate. Based on the health profiles of the city of Pematangsiantar, Maternal Mortality Rate from 2012-2016 experiencing ups and down, that is in 2012 as many as 11 deaths, then it decreased in 2013 to 4 deaths, and experienced an increase again in 2014 to 7 deaths, then it diclined again in the last two years, in 2015 there were 5 deaths and in 2016 there were 4 deaths. Purpose : To improve maternal and child health to give services sustainably and continuity of care to Mrs. C period pregnancy to become a family planning acceptor in accordance with the standards and authority of the midwife. Method : Continuity midwifery care and documentation with SOAP(Subjektif, Objektif, Asessment, Planning) management. Result : Mrs. C was 35 years old, multigravida, Gravidarium III, Giving Birth II, Abortion 0, gestational age is 30-32 weeks. The last menstrual period was in May,02nd 2017,and the estimated date of birth is in February, 09th 2018. During pregnancy, the ANC performed by the mother had mild anemia, this can be overcome by total bedreas and giving Ferum tablets. The process of labor is normally with, the weight 3400 gram, the body length 50 cm, the head circumference 33 cm, the chest size 34 cm, the arm circumference 12 cm, Apgar score 8/10, man, there is no congenital anormalies, there is a rupture of the perineum with grade I. At the time of post natal care and neonatal care no complications found. Mrs. C has been informed about the use of contraception, and mother choose method of family planning that is short time method injection 3 month. Conclution : In the case of Mrs. C, 35 years old with mild anemia and rupture perineum grade I. There is no gap between theory and practice. Services and care provided in accordance with standard of care and authory of the midwife.
