dc.description.abstractMaternal and child health service are things that need to be prioritized in the implementation of health efforts, because mother and children are vulnerable to family and surrounding conditions in general. Purpose to improve the maternal and child health status by applying continuity of care on pregnancy, maternal, postnatal, newborn and family planning. Continuous midwifery care and documentation of SOAP with midwifery management approaches in Mrs. M age 24 years, G1P0A0, HPHT 20-05-2018, TTP 27-02-2019, at 38 weeks gestation, fetal movements are first felt by the mother at the age of 16 weeks of pregnancy, and have received TT immunization twice. In the normal delivery process Mrs. M having undergone second degree perineal rupture suturing and no problems were found in the treatment of perineal wounds, babies born spontaneously with BB 3300 grams, PB 50 cm, LK 33 cm, LD 32 cm, male gender, with an apgar score of 9/10. The umbilical cord broke on day 6 and Mrs. M being a 3 month injection acceptor. The application of the mindset of midwifery care done to Mrs. M with continuity of care is expected to be a benchmark in providing midwifery serviceen_US
