Abstract

Background of problem : Women throughout their life cycle require monitoring both physiologically and pathologically. One of them is antenatal care, which is one of the preventive efforts of obstetric health care programs to optimize maternal and neonatal outcomes through a series of routine monitoring activities during pregnancy until family planning. Purpose : To provide midwifery care to Mrs. D 22 years of continuity of care starting from pregnancy, maternity, postpartum, newborns, and family planning in accordance with midwifery standards and midwifery care. Method : continuity midwifery case and documentation with SOAP management. Result : Mother aged 22 years, GIP0A0, HPHT 06-08-2018, TTP: 15-03-2019 After midwifery care is given to Mrs. D there is an anemia problem that has a low Hb level of 10.6 gr%, the mother looks pale and easily fatigued. . During pregnancy the mother experiences mild anemia, but at the end of pregnancy and at the next visit Hb Ny. D increased by 10.9 gr% of maternal anemia can be handled well. . Mothers are encouraged to consume lots of nutritious foods and regularly take Fe tablets. During labor the mother experiences rupture of the second degree perineum and Ny. D Infusion is given because it looks weak and slightly pale. Childbirth is born with BB 3200 grams and PB 50 cm and Apgar Score 8/10, male sex and care given according to the needs of the newborn. During childbirth, Mrs. D had no problems with any complications until Mrs. D became a family planning acceptor. Conclusion : Planning Ante Natal Care was found to be a problem for women with mild anemia, the management of consuming Fe 1x1 tablets and consuming nutritious foods. Delivery occurs in the second degree of perineal rupture, suturing of perineal rupture has been carried out and there is no problem. Postpartum and newborns have no complications. Mother has become a 3-month injection family planning acceptor
