Abstract

Background : In 2013 the Maternal Mortality Rate (MMR) in the world 210 per 100,000 live births and MMR in developed countries 16 per 100,000 live births. AKI in East Asia 33 per 100,000 live births, South Asia 190 per 100,000 live births, Southeast Asia 140 per 100,000 live births and West Asia 74 per 100,000 live births. Infant Mortality Rate (IMR) in developing countries 37 per 1,000 live births, and IMR in developed countries 5 per 1,000 live births. IMR in East Asia 11 per 1,000 live births, South Asia 43 per 1,000 live births, Southeast Asia 24 per 1,000 live births and West Asia 21 per 1,000 live births (WHO, 2014). Objective : The purpose of this paper is to provide comprehensive obstetric care for pregnant, maternity, postpartum, BBL and family planning in accordance with care standards and to use documentation with a midwifery management approach. Methods : The method used for ongoing Midwifery Care and documentation with SOAP management (Subjective, Objective, Assessment, Plan). Result : continuous midwifery for Mrs. S GIIPIA0 is 33 years old, gestational age 28-30 weeks obtained HPHT data: 05-05-2018, TTP: 12-02-2019. Examination of pregnancy visits for 4 times is 1 time on Tr. I, 1 time in Tr II, and 2 times in Tr.III. Examination of Hb 10.2 gr% and diagnosed with mild anemia. Mrs. E recommended consumption of 90 Fe tablets with nutritious foods. Labor occurs spontaneously. At delivery Ny. S there are no complications. The postpartum period is physiological in view of the bleeding experienced by the mother. Babies born healthy with PB: 50 cm, BB: 3700 g, female sex, and at the last visit informed of the use of contraception, the mother decided to become a DMPA (Depoprovera) injection contraceptive acceptor for 3 months. Conclusion : In this case, Mrs. S 33 years old with mild anemia there is no gap between theory and practice. Mothers are encouraged to consume nutritions food, Fe tablets and become KB acceptors
