Abstract

Postnatal and antenatal surveillance is essential in reducing maternal and perinatal morbidity and mortality. Antenatal surveillance provides benefits with the discovery of various abnormalities that accompany early pregnancy, so that it can be calculated and prepared for steps in the delivery aid. The purpose of this paper is to improve the degree of maternal neonatal health with continuity care starting from the period of pregnancy until becoming Family Planingacceptor.The report is prepared by using midwifery management with SOAP method. The standard of midwifery care is a reference apply the decisionmaking process and measures midwife. It is corresponding withauthority and its scope of practice in pregnancy, maternity, postpartum, newborn, and family planning or FP. Mrs. I, second gravid, once partus and never abortus. Pregnancy checks starting from the first trimester to the third trimester. ANC examination 8 times, delivery by midwives, 4 months childbirth visit, and newborn visit 4 times, and 3 kb injection. Psychosocial pregnancy is well received by the mother and family. When delivery Mrs. I undergoes second-degree perineal rupture and suture with cut-gut yarn using a baste technique. Maternal postpartum period is physiological seen from bleeding experienced by mother. The baby was born healthy with length of body: 49 cm, weight of body: 3200gr, male gender. The result of inspection we found that Mrs.I haveanemia problem, but it can be solve by giving tablets of Fe one tablets for one day and consuming nutritions food. Mrs.I has perineum rupture with second degree. And has been resolved well, without any danger or complication. The care provided is begun from pregnancy up to be a family planning acceptors in accordance with the standards of care and authoritymidwife
