Abstract

Background of problem : The supervision of pregnancy period, maternity, nerwborn baby, and family planning are urgently needed to reduce the maternal mortality and pain. Continuity of care is one of the effects to improve health during pregnancy until obstetrics management. Purpose : To give guard obstetrics to Mrs. A age 27 years old by contiinuity of care start in pregnancy, maternity, postnatal, newborn baby dan family planning by the standards of obstetrics care and obstetrics management. Method : Continuity midwifery case and documentation with SOAP management. Result : After the continuity of care to Mrs. A GII PI A0 there anemia to low that 10,5gr%. Mother look pale and easily exhausted. To overcome any complications during chhldbirth was give mother a minimum 0f 90 tablet Fe during pregnancy and feeds on ironrich food such as green vegetable, spinach, etc. the next visit with the mother’s Hb level increased and was normalis 12gr%. When the maternity Mrs. A doesn’t have complication but was give IV fluids because looked pale and weak. Maternity process newborn baby with woight 2900 grams and length 49 sm, apgar score 8/10, gender male. When the maternity Mrs. A doesn’t have complication but was give IV fluids because looked pale and weak. The care of nwebborn given as needed, there are no complication with baby and Mrs. A become Family Planning. Conclusion : In this case Mrs. A age 27 years old with anemia at the age of 26-28 weeks of pregnancy and has been given Fe tablet with continuity of care mother hasn’t anemia and become family planning
