Abstract

Back ground of problems:The implementation of health efforts in various areas is basically aimed at achieving the goal of health development to bring about the highest degree of public health through accessibility,affordability,and quality of health care and thus able to anticipate changes in development of problems and challenges to healthdevelopment themselves. Purpose: To provide care in obstetrics at Mrs. D age 27 years old with continuity of care started from pregnant women, maternity, childbirth, and newborn and acceptor family planning in accordance with the standards careof midwifery and midwifery management. Method: Continuity midwifery care and documentation by SOAP management. Result: Care in midwifery in a sustainable manner of Ms. D pregnancy with mild anemia does not lead to complications in pregnancy and chilbirth. The baby was born naturaly with the weight of 4000 gr, the length of 48 cm, apgar score 8/10 with woman gender. In the implementation of the care kala II, Mrs. P rupture and loose cord loose center but the problems can be resolved by doing hecting with baste technique and loosen the umbilical cord from the top of the head . Care on a newborn baby given according to the neeeds of baby’s. In a recent visit of the childbirth, has been informed about the use of contraception. The patient decided to become acceptor family planning implant. Conclusion: In the case of Mrs. D 27 years old with mild anemia, rupture perineum and loose cord loose center, it is found no gap betweeen theory and practice. The patient is recommended to consume nutrious foods, Fe tablet, and be acceptor family planning
