Abstract: Back ground of problems: The succes of effort mother healthy can we see from mother mortality rate indicator (MMR). MMR is mother mortality rate while pregnant, lab or puerperal, cause by pregnat,lab or, and puerperal or management but not because of another reaon. For example accidentor fall every 100.000 life labor. Purpose: To provide care in obstetrict at Mrs.M age 27 years old continuity of care started form pregnantwomen, maternity, childbirth, and newborn and acceptor family planning in accordance with the standards care of midwifery and midwifery management. Method: continuity midwifery care and documentation by SOAP management. Result: Care in midwifery in a sustainable manner of Ms.S with lab or procce baby born with the weight of 3500 gr, thr length of 48, apgar score 7/9 with woman gander. In the implrmrtation of the care kala II,Mrs. M rupture but the problems can be resolved by doing hecting with baste techique. Care on newborn baby given according to the needs of baby’s . in a recent visit of the childbirth, has been informed about the use of contraception. The patient deceded to become acceptor family planning implant. Conclusion: in the case of Mrs.M 27 years old with rupture perineum, it is found no gap betweeen theory and practice. The patient is recommended to consume nutrious foods, Fe tablet, and be acceptor family planning
