Abstract

Back ground of problems: In 2013 AKI in Indonesia reached 190 per 100,000 live births. When compared with Malaysia, the Philippines and Singapore, the figure is greater than the figures from these countries. AKB in Pematangsiantar City in 2016 increased to 6 per 1000 live births, compared to 2014 and 2015 of 4 per 1000 live births, this figure is still low and contributes positively in achieving the Renstra of Health Strategic Plan target for AKB in 2016 targeted 24 per 1000 live births Purpose: To provide care in obstetrics at Mrs. E age 32 years old with continuity of care started from pregnant women, maternity, childbirth, and newborn and acceptor family planning in accordance with the standards careof midwifery and midwifery management. Method: Continuity midwifery care and documentation by SOAP management. Result: Care in midwifery in a sustainable manner of Ms. E pregnancy and chilbirth. The baby was born naturaly with the weight of 3500 gr, the length of 48 cm, apgar score 8/10 with man gender. In the implementation of the care kala II, Mrs. E rupture but the problems can be resolved by doing hecting with baste technique. Care on a newborn baby given according to the neeeds of baby’s. In a recent visit of the childbirth, has been informed about the use of contraception. The patient decided to become acceptor injection family planning . Conclusion: In the case of Mrs. E 32 years old with rupture perineum it is found no gap betweeen theory and practice. The patient is recommended to consume nutrious foods, , and be acceptor family planning
