Abstract: | Background : Healty of mother and child is the most part to effort to increase health degrees into Sustainable Development Goal (SDG,s) in indonesia 2030. The indicator wiil reach the target if the maternal mortality rate, Neonatal Mortaliti retes and infant mortality rate decline and Family Planning;s acceptor can increase. Object : Given service midwifery continuity of care starts from the beginning of pregnancy, giving birth, postpartum, newborn baby until becomes as an acceptor family planning. Method : Study case method and documentation with SOAP management. Result: Mrs. O aged 21 years old, gravidarium I, giving birth O, abortion 0 in the three trimester gestation. The first contact when the age of pregnant was 29-31 weeks. When the deliveri’s time, the is complication with Msr.R the complication is Ruptur of the perenium level 2 and there iis no complication wih baby and normal baby care is done. Monitoring is done during the puerperium to assess the normality of the process experienced to become a family planing acceptor. Family planing method choose Mrs.R of short method is injection and done by injection KB 3 month. The state of the beby from birth to complication from neonatal period is monitored to see the possibility of problems that occur. Conclusion : The care from pregnancy to family planning acceptors in accordance with the standard of care and authority of the midwife |
