Abstract

Background : Health mother and child is a thing get priority in organization for health, because mother and child is susceptibel group to family and its area in general, so that the asessment of health status and performance of maternal and child health efforts is important to do. Purpose : Writing purpose is to increase health level mother and child by apling continuity of care to pregnant women, maternity, childbirth, and newborn and family planing. Method : The method used is continuity midwifery care and documenting by using Subjective Objective Assessment Plan management. The Result : Mrs. D age 26 years old, GIIIPIIA0, HPHT 15-05-2018, 7 visits, causing constapation, at gestasional age 28-30 weeks, can be solved tell to mother, to consume foods that are rich in fiber green vegetable and fruits ( papaya, banana, etc ). When procces normal childbirth, Mrs. D causing perineum rupture level ll, sewing and there was no problem in treating perineum wounds. Newborn baby weight : 3000 gram, length : 50 cm, gender : male, Apgar Score : 7/10. Baby was get braest feeding,, the umbilical cord broke on day 5. When puerperium Mrs. D feeling well. The lactation proccrs gang well and baby want to suckle, when the new born baby care there was no problem with weight : 3000 gram, length : 50 cm, lingual : 33 cm, bust :32 cm, apgar score : 7/10, gender : male and Mrs. D being an acceptor to a family planing a three month injection. onclusion : Implementing concept of midwifery care is conducted to Mrs.D with continuity of care, it is hoped to be barometer and basic direction in performing midwifery service.
