dc.description.abstractBackground : health service s during pregnancy,maternity, postpartum, period, intervals and care for newborns and documenting midwifery that has been carried out for pregnant,maternity,postpartum,neonatal,and family planning Objective : provide midwifery care continuity of care in pregnant ,postpartum, neonatal and family planning pregnant women using the midwifery management approach Method : the method used is continuous midwifery care and documentation with SOAP management Result : implementation of care for miss Y. GII PI AB0 HPHT 03-05-2018 4x visit to consume ferum tablets 1 time a dsy. Found several things that accompany the delivery process, namely there is complication,namely the mother had a second degree perenium rupture. Care for baby miss Y. with needs and no signis of danger complications. The last visit during purperium provides counseling for family planning and miss .Y decided to become a family acceptor planning a 3 month hormonal injection Conclusion : application of care continuty of care the midwife is expected to be able to reduce the maternal mortality rate (AKI) and infant mortality rate (AKB) and the achievement of family planning acceptors increasesen_US
