dc.description.abstractBackground : Maternal mortality and illness are still serious health problems in developing countries. According to the 2014 World Health Organization (WHO) the Maternal Mortality Rate (MMR) in the world is 289,000. The number of Maternal Mortality Rate (MMR) decreases from 4,999 in 2015 to 4912 in 2016 and in 2017 as many as 1712 cases. Purpose : To provide a comprehensive care service or continuity of care in performing its role sake of the health of mothers and children so as to reduce Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). Method : The method used is ongoing midwifery care and documentation with SOAP management. The results : Mrs. N is 28 years old, GII PI A0, gestational age 35-36 weeks, last menstrual period in July 15th 2018, date of birth in April 22nd 2019. There is no problem found of implementation midwifery care on Mrs. N. At the time of childbirth a healthy-born baby without any complications and no specific complications, only a second degree perineum rupture occurs and can be handled according to existing procedures. Perineal rupture experienced can be caused by a baby’s weight of 4.200 grams. Childbirth period N does not experience any complaints, the lactation process runs smoothly and the baby wants to breastfeed. Mrs. N became a family planning acceptor for 3 month injection. Conclusion : The care provided from the time of pregnancy to becoming a family planning acceptor, it runs normally. So the effort to improve health quality for pregnant women is to carry out comprehensive care or continuity of care so that it can reduce the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR).en_US
