Abstract

Method: Midwifery care and documentation with SOAP management. Result: Mrs. N is 37 years old, GIVPIIIA0 gestational age 24-26 weeks, HPHT 10-06-2018, TTP 03-17-2019. The mother feels the movements of her fetus at 16 weeks' gestation. During pregnancy, the ANC performed is found to be pregnant with Emesis Gravidarum and Breech Layout, but at the end of pregnancy Emesis Gravidarum and Mother Breech Position can be handled properly. Babies born spontaneously cried immediately, with JK PR, BB 3300 gr, PB 49 cm, Apgar score 8/10, no congenital abnormalities. During labor the mother does not experience perineal rupture. In the implementation of childbirth care there was no problem found. Childbirth period N runs normally, no complications are found. There was no problem with BBL. At the end of the postpartum visit KB counseling was conducted and the mother chose to be a 3-month injection KB acceptor. Conclusion: Midwifery care is provided from pregnancy to becoming a family planning acceptor in accordance with midwifery care standards and the authority of a midwife.
