Abstract

Background : During pregnancy, pregnant women must carry out antenatal care check at the health care facility at least four times. The standard time of service is to ensure protection for pregnant women and fetus in the form of early detection of risk factors, prevention and management of complications of pregnancy. Purpose : To provide midwifery care to Mrs. R, 29 years old in continuity of care starting from pregnant women, childbirth, puerperal, newborn baby and family planning acceptors in accordance with the standards of midwifery care and obstetric management. Method : continuity of care with Varney documentation continue SOAP in accordance with midwifery care standart. Results : Continuous midwifery care for Mrs. R 29 years, second pregnancy have given birth once and never abortion. Birth of spontaneous birth with 3400 gram of weight, body length of 51cm, 33cm of head circumference, 34 cm of chest size, APGAR SCORE: 9/10 with male sex. In the implementation of care at the second stage Mrs.R have second-degree perineal rupture but the problem can be resolved by doing a second degree hecting with the brighter technique. Care at the BBL was given as needed and there were no signs of danger or complications found in the baby. In the last visit of the puerperium has been informed about contraceptive use and the mother decides to become an injectable family planning acceptor. Conclusion : In this case, Mrs. R 29 years old with second grade perineum and there was no gap between theory and practice. The services and care provided were in accordance with the midwife's care and authority standards
