dc.description.abstractPostpartum can occur with various complications such as bleeding due to uterine atony, retained placenta, and ruptered perineum. According to WHO there are 2,7 million cases of perineal rupture in maternal, estimated to reach 6,3 million in 2050. In Asia perineal rupture in society 50% of the incidence of perineal rupture in the world. To improve the maternal and child health status by applying continuity of care on pregnancy, maternal, postnatal, newborn and family planning. Midwifery continuity of care and SOAP management. Mrs. S (31 y.o), G4P3A0, HPHT 28-05-2019, 3 visits, experienced physiological Insomnia at 34-35 weeks` gestation, can be overcome by telling the mother to get enough rest and get enough sleep, at night 8 hours and at noon 1 hour, and often Urinating physiologically at gestational age 35-36 weeks, can be overcome by telling the mother to drink 2-3 hours before going to bed at night, multiply drinking during the day, when Urinating make sure the bladder is completely empty. In laborn Mrs. S is experienced coils of umbilical cord,but it can resolved with Standard Operasional Prosedur and rupture perineum grade II, perineal wound suturing and no problems found. The baby was born spontaneously and gets the breast milk. In the postnatal period, Mrs. S has no problem, breastfeeding happens smoothly happen. In the Newborn baby care there is no complication, weight 3500 gr, height 48 cm, head circumference 33 cm, chest size 32 cm, apgar score 8/10, male sex, the umbilical cord is loose at day-6 and Mrs. S becomes Family planning acceptor of 3 month injection type. The application of mindset to continuity of care taken on Mrs. S are expected to be a benchmark or guidelines to give midwifery careen_US
