Abstract

Background : The Health Profile of North Sumatra Province in 2018 shows that the assessment of the implementation of health services for pregnant women can be done by looking at the coverage of K1 and K4. K1 coverage is the number of pregnant women who have received antenatal care for the first time by health workers, compared to the target number of pregnant women in one work area within one year. Whereas K4 coverage is the number of pregnant women who have received antenatal care according to standards at least four times according to the recommended schedule in each trimester, compared to the target number of pregnant women in one work area within one year. Objective : The purpose of writing is to improve the degree of health of mother and child by applying Continue of Care care to pregnant, childbirth, postpartum, newborn and family planning care. Method : The method used is ongoing midwifery care and documentation with SOAP management. Results : Ny. S 27 years old G3P2A0 HPHT 05-15-2019 4 visits, during the Mrs S process there were no perineal wounds. Babies are born spontaneously and can be breastfed. The puerperium Ny. S hasn't experienced any complaints. The lactation process went smoothly at the care of the newborn with no complications. BB: 3255 gr, PB: 49 cm, LK: 33 cm, LD: 34 cm. Apgar Score 8/10, Male sex, Cord umbilical on day 6 and Ny. S becomes a 3-month KB acceptor. Conclusion : Midwifery care is given starting from pregnancy to being a KB acceptor, according to the midwifery care standards and the authority of a midwife by implementing continuity of care.
