Abstract

Backgrounds: Maternal Mortality Rate (MMR) is one indicator to see the success of maternal health efforts. Anemia in pregnant women can increase the risk of premature birth, maternal and child mortality, and infectious diseases. Iron deficiency anemia for mother can affect the growth and development of the fetus / baby during pregnancy and afterwards. One of the efforts to improve the quality of maternal and child health services is to carry out ongoing care or continuity of care. Purpose: Provide care in obstetrics on Mrs. J (39 years old) with continuity of care started from pregnancy, maternity, postnatal, newborn and family planning acceptor in accordance with the standards care of midwifery and midwifery management. Method: Midwifery continuity of care and documentation by SOAP management. Result: After midwifery care to Mrs. J was found Mild Anemia, but it was handled with improvement of the pregnancy nutrients. In the theory of pregnancy with anemia can lead to bleeding and deaths during maternity and postnatal. The midwifery of care on Mrs. J pregnancy with mild anemia did not lead to complications in pregnancy and maternity. The baby was born spontaneously with the weight 3200 gr, length 50 cm, apgar score 8/10 with female gender. In the implementation care of kala II, Mrs. J experienced rupture but the problem solved by doing hecting with baste technique. The care on a newborn was given according to the needs of baby. In the recent postnatal visit, had been informed about usage of a contraception. The patient decided to becomes family planning acceptor of injection one time in three months. Conclusion : The care from pregnancy to family planning acceptors in accordance with the standard of care and authority of the midwife
