Abstract: Background of problem : The Health Profile of the Province of North Sumatra in 2017 showed that an assessment of the implementation of health services for pregnant women can be done by looking at the coverage of K1 and K4. The Infant Mortality Rate is one indicator of sustainable development goals in 2030. Purpose : The purpose of writing is to improve the health of mothers and children by applying continuity of care to pregnant women, maternity, postpartum, newborns, and family planning. Method : The method used was ongoing midwifery care and documentation with SOAP management. Result : Mrs. S aged 29 years, GII PI A0 gestational age 30 - 32 weeks, HPHT 14-04-2019, TTP 21-01-2029. During pregnancy, from ANC performed by mothers experiencing gestational hypertension, this is overcome by nutritious food and the administration of nifedipine drugs. 4 visits, during the normal delivery process Mrs. W did not find any wounds on the perineum. Babies born spontaneously BB 2900 gr, PB 49 cm, LK 33 cm, LD 32 cm, female gender, apgar score 6/10. The baby got breast milk, the cord was puput on day 6. In the care of the newborn there are no complications. Childbirth period Mrs. S did not experience any complaints, the lactation process went smoothly and the baby wanted to breastfeed. Mrs. S became a KB acceptor for 3 month injection. Conclution : The application of the mindset of midwifery care done to Mrs. W with continuity of care is expected to be a benchmark and guideline in providing midwifery services
