Abstract: Background: The Health Profile of North Sumatra Province in 2018 shows that an assessment of the implementation of health services for pregnant women can be done by looking at the coverage of K1 and K4. Infant Mortality Rate is one indicator of sustainable development goals in 2030. Purpose: The purpose of writing is to improve the health status of mothers and children by applying continuity of care care to pregnant, childbirth, postpartum, newborn, and family planning care. Method: The method used is ongoing midwifery care and documentation with SOAP management. Results: Ny. V age 30 years old, GIII PII A0 gestational age 30-32 weeks, HPHT 05-20-2019, TTP 02-27-2020. During pregnancy, from ANC by anemic mother, this is overcome by nutritious food and administration of the drug fe. 4 visits, in the normal delivery process Ny.V found no injuries to the perineum. Babies born spontaneously BB 3400 gr, PB 49 cm, LK 33 cm, LD 32 cm, female sex, apgar score 9/10. Babies get breast milk, umbilical cord puput on day 6. In the care of newborns found no complications. The puerperium Ny. V did not experience any complaints, the lactation process went smoothly and the baby wanted to breastfeed. Mrs. V is a KB acceptor for 1 month. Conclusion: The application of the midwifery care mindset carried out at Ny.V with continuity of care is expected to be a benchmark and a guideline in providing midwifery services
