Abstract

Background: Assessment of the implementation of maternal health services can be done by looking at running K1 and K4. During 2006 to 2018, health services for K4 pregnant women increased. Purpose: To provide midwifery care to Mrs. R who was 30 years old with continuity of care starting from pregnancy, childbirth, postpartum, newborn, until become the family planning acceptor in accordance with the standards of midwifery care and obstetric management. Methods: Midwifery care is managed and documented with SOAP management. Results: Ongoing midwifery care for Mrs. R with spontaneous delivery of newborns with BB 3100 grams, PB 49 cm, apgar score 8/10, has male sex. During the second stage of care, Ny. Resolve level II, but the problem can be overcome by doing level II and the bourgeois technique. Care for LBW was given as needed and there is no evidence of danger or complications found in infant. At the last postpartum visit was informed of contraceptive use, the mother decided to become a 3 month injection acceptor. Conclusion: In this case Mrs. R was 30 years old with perineal rupture, and there is no conflict between theory and practice
