Abstract

Propose : To provide midwifery care to Mrs. E 26 years old in continuity of care starting from pregnant women, childbirth, postpartum, newborn and family planning acceptor in accordance with the standards of midwifery care and obstetric management. Method : That method used are continuity of care and documentation with SOAP management. Results : In martenality, from doing antenatal care getting that maternal with anemia, but in last maternal, that anemia and her waist pain could be resolved. Birth of spontaneous birth with male sex, 3200 gram of weight, 49 cm of body length , APGAR SCORE 8/10, there were not kongenital difference. At her confinement not experience of perineal rupture. At doing puerperal care there was not any problem. puerperal Mrs. E doing well, never getting troubles.At newborn baby there isn't any problem. in The last visiting of puerperal, I did Planing Of Family konseling and her choose to be injectable three month Planing Of Family acceptors. Conclusion : Midwifery care that given since maternity until Planning Of Family acceptors in accordance with the midwife's care standards and authority standards. In this case, Mrs. R 29 years old with second grade perineum and there was no gap between theory and practice. The services and care provided were in accordance with the midwife's care and authority standards
