Abstract: Background: The results of the 2017 Indonesian Demographic and Health Survey (IDHS) showed that the AKN was 15 per 1,000 live births, the IMR 24 per 1,000 live births, and the AKABA 32 per 1,000 live births. Mortality rate has reached the Sustainable Development Targets (TPB / SDGs) of 2030 which is 25 / 1,000 live births and it is expected that AKN can also reach the target of 12 / 1,000 live births. Objective: To implement midwifery care that continuity of care for pregnancy, childbirth, postpartum, newborns and family planning in accordance with care standards using SOAP documentation with obstetric management approaches. Methods: Continuous midwifery care and documentation with SOAP management. Results: The delivery of baby was spontaneous with the weight 3300 grams, the length of 49 cm, apgar score 8/10, with female sex. BBL care was given according to the requirements and not found any complications in infant. In the last visit had been informed about the use of contraception. The mother decided to accept injecting contraception once every 3 months. Conclusion: on this case Mrs. D had age 30 years old with normal childbirth there was no gap between theory and practice. It is also recommended to consume nutrition foods and become acceptor of family planning
