Abstract

Background : Target reduction in MMR is determined through three models of Average Reduction Rate (ARR) or the rate of decline in average maternal mortality. Of the three models, the Ministry of Health uses the second model with an average reduction of 5% per year as a performance target. Based on this model it is estimated that in 2030 the MMR in Indonesia will drop to 131 per 100,000 live births. Purpose : The aim is to improve the continuity and quality of mother and child which carried out with an approach ( Continuity Of Midwifery Care) because it will have a significant impact on the survival and quality of life of mothers and children. Method : The method used is the continuity of midwifery care and documentation with SOAP management. Result : Mrs. B was 31 years old, G3 P2 A0 33-35 weeks gestational age, HPHT 03-10-2019, TTP 12-17-2019. The mother felt the fetal movements at 16 weeks' gestation. During pregnancy, from the ANC conducted no abnormalities or problems were found from the results of the examination. The baby who born spontaneously cry with JK PR, BB 3700 gr, PB 48 cm, and there is no congenital abnormalities. During childbirth mother faces perineum rupture at first degree, but the midwife can handle very well . In the implementation of postpartum care there were no problems in the perineum scarring area. Mrs. B postpartum walked normally, no complications were found. In BBL there were no problems. At the end of the postpartum consultation, family planning counseling was performed too and the mother chose to become a 3-month injecting family planning acceptor. Conclution : Midwifery care was given starting from pregnancy to becoming a family planning acceptor according to the midwifery care standards and the authority of a midwife. If the patient experiences a complaint, immediately come to the nearest health-care provider
