dc.description.abstractBackground of problems: Maternal Mortality Rate (MMR) is one indicator to see the success of maternal health efforts. In addition to assessing maternal health programs, this indicator is also able to assess the degree of public health, because of its sensitivity to improve health services, both in terms of accessibility and quality. In general there was a decrease in maternal mortality during the 1991- 2015 period from 390 to 305 per 100,000 live births (Ministry of Health, 2018). Purpose: To provide midwifery care to Mrs. J 37 years old in continuity of care starting from pregnant women, childbirth, postpartum, newborns and birth control according to the standards of midwifery care and obstetric management Method: Continuity midwifery care and documentation by SOAP management. Result: Care in midwifery in a sustainable manner of Mrs. J pregnancy with mild anemia did not lead to complications in pregnancy and childbirth. The baby was born naturally with the weight of 3450 gr, the length of 50 cm, apgar score 8/10, male sex. In the implementation of the care kala II, Mrs. J rupture but the problems can be resolved by doing hecting. Care on a newborn baby given according to the neeeds of baby. In a recent visit of the childbirth, had been informed about the use of contraception. The patient decided to become acceptor family planning implant. Conclusion: In the case of Mrs. J 37 years old with mild anemia,and rupture perineum, it was found no gap between theory and practice. The patient was recommended to consume nutritious foods, Fe tablet, and be acceptor family planningen_US
