Abstract: Background: Maternal and child health is very important, mothers and children are family members who need to get priority in organizing health efforts because they are vulnerable group. This is related to the phases of pregnancy, childbirth and the puerperium for mother and the phase of growth and development for children. Purpose: Provide contunuity care for obstetric care to Mrs. A starting from pregnancy, childbirth, childbirth, neonates, and family planning by taking a midwifery management approach. Methods: Continuous midwifery care and documentation with SOAP management. Results: Continuous midwifery care to Mrs. A in pregnancy has no problem. The baby was born spontaneously with BB 3200 gr, PB 50 cm, apgar score 8/10 with female sex. During the second stage of care, Mrs. A experienced a degree II rupture and performed hecting with the jeluda technique. Newborn care is given as needed and there are no signs of danger or complications found in the baby. At the last visit Table 3.1 APGAR Value in newborns. At the time of childbirth informed about the use of contraception, the mother decided to become a 3-month injectable family planning acceptor, with the reason to continue to smoothly feed her baby. Conclusion: In this case Mrs. A. was 25 years old with perineal rupture, there was a gap between theory and practice. The baby is was good health and has received HB0 immunization. Mother has received injection of depoprovera KB 3 mth
