Abstract: Background : The success of mother health efforts can be seen from the indicator of mother mortality rate. Anemia in pregnancy affects the condition of mother with haemoglobin below 11 g/dL in first and second trisemester or level <10,5 g/dL in second trisemester. The limit value happens because of hemodilution levels, especially in the second trisemester. The effort to decrease the mortality of mother, baby and child, one of them is doing the continuity of care. The purpose : Given service midwifery continuity of care starts from the beginning of pregnancy, giving birth, postpartum, newborn baby until becomes as an acceptor family planning. Method : Study case method and documentation with SOAP management. The results : Mrs. R, age 32 years, G3P2A0, HPHT 18-06-2018, TTP 25-03-2019. Four visits in the third trimester, experience physiological anemia at 28-30 weeks gestation. Can be overcome by taking Fe tablets once a day. During normal birth, Mrs. R has a perineum rupture level 1, suturing was done, and no found any problems in the treatment of perineum wouds. The baby born spontaneously, weight 2800 gr, height 50 cm, male, apgar score 7/10. The lactation process runs well and Mrs. R become a family planning acceptorwith Depo-injection. Conclusion : The care from pregnancy to family planning acceptors in accordance with the standard of care and authority of the midwife
