Abstract: Background: Maternal health services must meet a minimum frequency every quarter. Assessment of the implementation of maternal health services can be done by looking at the approval of K1 and K4. During 2006 to 2018, health services for K4 pregnant women increased 79.63% to 88.03%. Objective: To apply midwifery care to Mrs. S. Age 25 years of continuity of care in accordance with the standards of midwifery care by requiring management Methods: Midwifery care is managed and documented with SOAP management. Results: After midwifery care was given to Mrs. Returning 25 years, GI P0 A0 28- 30 weeks gestational age, HPHT 05-02-2019, TTP 02-09-2020. The mother eats fetal movements at 12 weeks' gestation. During pregnancy, ANC can be found from pregnancy with anemia, but at the end of the pregnancy maternal anemia can be well received. Babies born spontaneously cry immediately, with JK LK, BB 3200 gr, PB 49 cm, Apgar score 8/10, no congenital abnormalities. At the time of labor the mother failed perineal rupture. In the implementation of childbirth care is not found any problems or normal, and no complications were found. In BBL no problems were found. At the end of the postpartum visit a family planning counseling was performed and the mother chose to become a 3-month injecting family planning acceptor. Conclusion: In this case Ny.S was 25 years old with mild anemia. Mothers are advised to consume nutritious food, Fe tablets and become a KB acceptor.
