Abstract

Back ground of problems: In 2018 AKI in Indonesia reached 830 per 100,000 live births. AKI in devolopment . countries 230 per 10.000 live births and AKI in developed countriess 239 per 10.000 live births. AKI in east Asia 33 per 10.000 live births. AKI in indonesia reached 305 Per 10.000 live births . while compared to Malaysia , the Philipine and Singapore , the figure was greater. Wheare as malayasia AKI was 44 per 10.000 live birth and singapore 7 per 10.000 live birth The number of maternal deaths in the city of pematang siantar from 2012- 2016 Experince ups and downs, namely in 2012 as many as 11 deaths , then decreased in 2013 to 4 deaths , and Experience an increase again in 2014 to 7 deaths, then decreased again in two last year ( in 2015 amounted to 5 deaths and in 2016 amounted to 4 deaths. If seen per district, the districts with the highest number of maternal deaths from 2012- 2014 were north siantar and siantar martoba each of deaths . Purpose: To provide care in obstetrics at Mrs. M age 28 years old was continuity of care started from pregnant women, maternity, childbirth, and newborn and acceptor family planning in accordance with the standards careof midwifery and Method: Continuity midwifery care and documentation by SOAP management . Conclusion: In the case of Mrs. M ,28 years old was found no gap betweeen theory and practice. The patient is recommended to consume nutrition foods,, and be acceptor family planning
