Abstract: Backgro und: Maternal mortality rate is the ratio of maternal mortality during pregnancy, giving Birth, puerperium, newborn period, caused by pregnancy, giving Birth, puerperium, Newborn period, or its management but not due to other causes such as accidents or falls in every 100.000 live births. In general, there was a decrease in maternal mortality during 1991-2015 period from 390 to 305 per 100.000 livebirths. Aim: To implement midwifery care continuity of care for pregnant women, childbirth, newborn and family planning according to midwifery care and obstetric management. Method: Ongoing midwifery care and documentation with SOAP management. The Results: After doing midwifery care to Mrs. W with a toothache. The theory of pregnancy with dental health disorders can lead to premature fetus and low birth weight. Ongoing midwifery care for Mrs. W pregnancy with mild anemia did not cause complication in pregnancy, childbirth, or the puerperium. The process of childbirth is born spontaneously with a weight Of 3000 grams, thelength of The baby is 49cm, apgarscore9/10,withmalegender. During the second stage of care Mrs. W did not experience rupture. Care for newborn birth was given as needed and there were no signs of danger or complications found in the baby. During the last postpartum visit to be informed about the use of contraception, the mother decided to become a family acceptor of an injectable injection plan once on 3 months.
