Abstract

Mother and baby are family member who needs to get priority in organize health efforts, because mother and child are members who are susceptible. This is related to the phase of pregnancy, childbirth and post-partum maternal and child growth and development phase. Application of the midwifery care with continuity of care in pregnancy, postnatal, newborns and family planning. According to the standard of midwifery with technique documentary subject object assessment planning with the/ midwifery management approach. Midwifery care with continuity of care using management subject object assessment planning in Mrs. I GIVPIIIA0 during pregnancy in the trimester II-III, with ANC 4 times and newborn visits 3 times, post natal care with perineal rupture level II, newborn with weight of 3600 gram, body length 50 cm, mothers with premium returns treatment of postpartum mother with perineal rupture, and implant KB installation. In labor process there were perineum rupture grade II giving care by wound using cut gut and giving medicine antibiotic and in three weeks the scars already recovered, there was nothing bleeding, the newborn baby with appreance pulse grimacy activity respiration 8/10. Implementation of post-partum care have no vi problem. During the interpal period the counseling was done, the mother decided to become an implant KB acceptor. To Mrs. I was applied continuity of care which is comprehensive based on obstetric knowledge so the maternity problem can overcome, without any complication. Midwifery care to Mrs. I was carried out continuity of midwifery care pregnancy to become a family planning acceptor according to midwifery standards.
