Abstract: | Background: Maternal mortality and morbidity remains a serious health problem in developing countries. According to the World Health Organization (WHO) in 2014, the maternal mortality rate (MMR) in the world was 289,000. Some countries had quite high MMR such as 179,000 Sub-Saharan Africans, 69,000 South Asians, 16,000 Southeast Asians. One of the efforts to reduce maternal, infant and child mortality is to carry out continuous care (Continuity of Care). Purpose: Provide ongoing midwifery care from pregnancy, childbirth, postpartum, ewborn and family planning Method: Case study method and documentation with SOAP management. Results: 32-year-old Mrs U, G2P1A0, HPHT 05-15-2019, TTP 02-22-2020. 4 visits during pregnancy. In the process of delivery Mrs.U’s amniotic fluid was cloudy and green. Mrs. U had a second degree perineal rupture, had a suture done and no problems were found in the treatment of perineal wounds. The baby was born spontaneously BB 3700 gr, PB 50 cm, female sex, apgar score 7/10. The lactation process went well and Mrs. U became an implant KB acceptor. Conclusion: The care provided from pregnancy to becoming a family planning acceptor is in accordance with the standards of care and the authority of the midwife. |
