dc.description.abstractBackground : Mothers and children are family members who need to get priority in the implementation of health efforts, because they are a vulnerable group to family conditions. This relates to the phase of pregnancy, childbirth and postpartum in the mother and the phase of growth and development in children. Purpose: To provide midwifery care to Mrs. I was 29 years old with continuity of care for pregnant women, childbirth, postpartum, newborns and family planning using a midwifery management approach. Methods: Continuous midwifery care and documentation with SOAP management. Results: Midwifery care starting during pregnancy, childbirth, postpartum to becoming a family planning acceptor did not have any significant problems. The delivery of the baby was born spontaneously with a weight of 2900 g, PB 49 cm, Apgar score 9/10 with male sex. Newborn care was provided as needed and no signs of danger or complications were found. During the puerperium, the mother does not mobilize enough so that it affects the time of uterine involution. Mother became a family planning acceptor at 4 weeks postpartum with an implanted family planning acceptor. Conclusion : Continuity of care has a positive impact on the health of Mrs. I and the baby she gave birth toen_US
